New and renewing student members must complete the Emergency Medical Authorization Form, the Field Trip Permission/Photo Use Consent Form, and the Health History and Examination Form. If you are joining online, please download, print, complete and mail these forms after you have completed payment. If you are joining by mail, please include these completed forms with your membership application.
